Have you discussed this problem with your immediate supervisor? YES x NO NATURE OR DESCRIPTION OF THE PROBLEM:
Today after my legal visit with Dolores of disability rights center Inc. Sgt. Carrol escorted me back to my barracks making rude obnoxious remarks about my footwear stating next time he wouldn't allow me to go to visit in the only footwear I have. This was a legal visit and his actions are purely out of retaliation.

WHAT DO YOU WANT TO HAPPEN TO SOLVE IT?
To be free from retaliatory harassment and threats by staff over seeking legal assistance over the very footwear the officer is harassing me over.

Inmate Signature John signed the form. Date August 15, 2000

IS THIS AN EMERGENCY SITUATION? YES x NO If so, why? (Provide Explanation) Yes, the state is obviously seeking revenge for my going to outside sources to acquire medical assistance.

Have you discussed this problem with your immediate supervisor? YES x NO NATURE OR DESCRIPTION OF THE PROBLEM:
I was seen by Dr. Mitchell over my hip. He prescribed "something" but I never received it (2 weeks ago).
Last week on approximately April 8, 2001, I put in another sick call so I could obtain treatment for my hip; it is very painful and my medical needs are obviously being ignored on purpose.

WHAT DO YOU WANT TO HAPPEN TO SOLVE IT?
To receive proper medical treatment; it is the result of 3-1/2 years of neglect by C.M.S. over orthopedic footwear that has caused my hip to slowly deteriorate.

Inmate Signature John signed the form and put his inmate number. Date April 16, 2001

IS THIS AN EMERGENCY SITUATION? YES x NO If so, why? (Provide Explanation) Nobody will sign for the grievance; I have no option but to put it in the mail.

WARDEN'S/CENTER SUPERVISOR'S DECISION

I have determined that your grievance is a medical matter. I have forwarded your grievance to the Medical Administrator who will provide a written response, and/or will interview you within twenty working days of the date I received your grievance. Should you receive no response within this time frame, or the response that you received is unsatisfactory, you may appeal to the Deputy Director for Health and Correctional programs. If you have medical needs that you believe are urgent, put in a Sick Call Request, or send a Request for an Interview to the Medical Administration.

INMATE'S APPEAL
If you are not satisfied with this response, you may appeal this decision within five days by filling in the information requested below and mailing it to the appropriate Deputy/Assistant Director. Keep in mind that you are appealing the decision to the original complaint. Do not list additional issues which are not a part of your complaint.

WHY DO YOU NOT AGREE WITH THE RESPONSE? You're a rocket scientist; you, too, notice that the grievance is medical. At least you read it for once instead of having inmates do your job for you. Who that you know can wait 20 working days after they develope a medical need; it's called a stall tactic to avoid the issue like it has been avoided for 3-1/2 years which created this problem with my hip. I will not accept your excuses and want the problem fixed. To force me to wait 20 working days in addition to this process is cruel and inhumane.

Have you discussed this problem with your immediate supervisor? YES x NO NATURE OR DESCRIPTION OF THE PROBLEM:
I have been trying for 2 to 3 weeks now to obtain medical treatment for my right hip; after being denied footwear (medical) 3-1/2 years my hip has developed problems; I need to have footwear but I also have hip damage as a residual from 3-1/2 years of neglect. Nurse Milton refused to give a sick call which further denies treatment.

WHAT DO YOU WANT TO HAPPEN TO SOLVE IT?
I want medical treatment. Is that asking too much? I want C.M.S. who is responsible to respond to my medical needs and not wait an additional 3-1/2 years to do so.

Inmate Signature John signed the form and put his inmate number. Date April 18, 2001

IS THIS AN EMERGENCY SITUATION? YES X no If so, why? (Provide Explanation) I am in pain which is extreme at times.

Have you discussed this problem with your immediate supervisor? YES NO John wrote "irrelevant" NATURE OR DESCRIPTION OF THE PROBLEM:
I have a serious problem with my right hip due to the malicious denial of proper medical care for 3-1/2 years. Today the so called doctor told me to come back in 3 months and I can have some aspirin. This is not medical treatment; this is a pacifier to avoid treatment.

WHAT DO YOU WANT TO HAPPEN TO SOLVE IT?
How about some legitimate medical treatment!! The mentality that an aspirin fixes all is ludicrous. I want to see somebody about this issue immediately!

Inmate Signature John signed the form and put his inmate number. Date May 16, 2001

IS THIS AN EMERGENCY SITUATION? YES x NO If so, why? (Provide Explanation) It is a medical problem which is getting worse; it has reached a point of cruel and inhumane proportions.

Have you discussed this problem with your immediate supervisor? YES x NO NATURE OR DESCRIPTION OF THE PROBLEM:

After having me sign for my aspirin the so called nurse threw it in the floor and used her foot to kick it under the door, claiming that to throw meds in the floor is somehow okay. This is pure laziness, highly unsanitary, cruel and inhumane.

WHAT DO YOU WANT TO HAPPEN TO SOLVE IT?
Fire this no account piece of garbage delivering the meds. I do not have any meds because she threw them on the floor.
