NAME (Please print) John Caldwell ADC# 09188 BARRACKS 14-419 JOB ASSIGNMENT A.S.Utility

Have you discussed this problem with your immediate supervisor? YES x NO NATURE OR DESCRIPTION OF THE PROBLEM: COI Hal, COI Burger, Sgt Canfield, and Lt. Toomes all refused me access to my scheduled medical ... [indistinguishable word] because all I have to wear is shower shoes; this is blatant intentional denial of medical care over what footwear I do or don't have.

WHAT DO YOU WANT TO HAPPEN TO SOLVE IT? This deliberate action has caused me undue pain and suffering by the blatant denial of medical care. Somebody needs to revise their sense of priorities as a medical appointment overrides any ridiculous aspect of what is allowed as footwear to go down the hallway.

Inmate signature John signed the form and placed his inmate number. Date April 25, 2000

IS THIS AN EMERGENCY SITUATION? YES x No If so, why? (Provide Explanation) I am maliciously being denied medical care over some footwear style prescribed by staff; medical is priority.

WARDEN'S/CENTER SUPERVISOR'S DECISION

I do not find staff denied access to the infirmary, as you were told you had to wear canvas or boots. If you had neither, then you could go in shower shoes.
Records
indicate you were seen in the infirmary on April 26, 2000.

If you do not agree with response, you may appeal my decision to the Appropriate Assistant Director with ten (10) working days.

INMATE'S APPEAL

If you are not satisfied with this response, you may appeal this decision within five days by filling in the information requested below and mailing it to the appropriate Deputy/Assistant Director. Keep in mind that you are appealing the decision to the original complaint. Do not list additional issues which are not a part of your complaint.

WHY DO YOU NOT AGREE WITH THE RESPONSE? All (4) of these staff members refused to take me to the infirmary for exactly the same reason, the new policy on footwear in the hallway, all I have is shower shoes. All (4) refused solely on that basis. Now to the alleged treatment mentioned by Warden Round of April 26, 2000, this is an out and out "lie" unjustifiable story with malicious intent, check your barracks log book, infirmary, look at barracks cameras, hallway cameras, and many other sources. I have not got fair treatment or even so much as been able to reschedule an appointment since this malicious denial of medical treatment. Meanwhile, I must suffer extreme pain day after day while the state makes excuses for its malicious actions and tells ... about it. I am in intense pain. This is wrongful, cruel, and under suspicion.

Note from John to Angela

I am no longer in the infirmary. Why you might ask is because I demanded the right to write and mail legal letters and personal letters. I was denied access to any form of writing materials. Finally on the 7th day, they allowed me to file a grievance C.O.I Rain...allowed me to file it, the next morning "today" they kicked me out of the infirmary. The only treatment received was blood pressure taken, temperature taken, and given an antibiotic "Flagill" (check spelling) for five days. Today in a last ditch effort to justify an excuse/diagnosis or to simply act as if they know what they're doing, they said he must have hemmorhoids. I do not have any ... hemmorhoids. I am in the hole with 30 days to do. My last day is July 26 [2000]. I could get out sooner, but they're mad that I demanded my legal right to write my attorney. Their excuse of treatment was bad, really bad. They left a bag in the cell (biohazard) this bag had bloody toilet paper in it and was in that cell/infirmary room the entire 7 days I was there. Yes, it smelled severely like a dead decomposing body, the ... thing stunk. I want you to write FOIA enquiry on (3) people, the (3) names are James Cheney, John Alwood, Gail Neal, a 4th name is Sherry Little. Write an FOIA on each one individually; mail these FOIA individually to: Arkansas State Medical Board, 2100 Riverfront Drive, Suite 2..., Little Rock, AR 72202-1793. You ask for copies of the license to practice medicine and for if they ever had a restriction on their license and why. Look at the ones I already sent you; I think my letter to ask questions is still in one of them. Look and see.
